ATTENDANCE FORMS
General Attendance Information
- Regular attendance at school is very important to the progress of our students. They must be in school every day unless they are ill or have a medical or dental appointment.
- California State Law requires that all students returning from an absence have an absence note or a verification card signed by a parent or guardian, explaining the date(s) and reason for the absence. Students must take these notes to the Attendance Office.
- This is a CLOSED CAMPUS. Students are not allowed to leave school for any reason without a permission slip from the Main Office.
- Under recent California State Legislative actions, schools only receive operational funds when students are actually in school and in class. Only when students come to school every day, does the State of California generate funds to Robert Frost Middle School.
- It is our goal to achieve actual in-seat attendance of 96% on a monthly basis.
- There is a direct correlation between excellent attendance and academic achievement. It is imperative that students are in school every day.

- Students should be on time to school and to their classes. If a student receives a “tardy unexcused” to school, consequences of Nutrition Detention, Paper Pick-Up may be assigned. Chronic tardies may result in consequences including but not limited to Lunch Detention or Saturday School.

Changes in Student Data
- All changes in phone numbers, addresses, or emergency contacts must be made in writing.
- Address change notice must be submitted with a gas/electric bill displaying the parent’s name and the new address. If the bill is not in the parent’s name, the owner of the property must come to their appropriate Grade Level Office with their bill and a photo ID to sign a formal statement verifying the residency of the student.
- Changes reported to the Grade Level Office will be made upon sufficient documentation and become effective in all computer systems within one week of the reported change.

Absence Policy
- Students are only excused for absences for illness, doctor/dental appointments, religious observance, and court appearance.
- Students are required to submit a parent note upon return from every absence.

Tardy Policy
- Students who arrive late should report directly to Homeroom and submit a parent note to the teacher to be excused.
- Students who arrive late to Homeroom without a note from their parent will be tardy unexcused.
- Students who arrive after Homeroom is dismissed to the next period must report directly to their
Grade Level Office with a note from their parent explaining the reason for the absence.

Early Release During School Hours
- Students bring a parent note to their Grade Level Office requesting early release. The student will be instructed on how to leave class in time to meet parent in the office. If someone other than parent will pick up the student, the person’s name must be included in the note.
- Adults over 18 years of age must either be the legal guardian or on Emergency Card to pick up a student. A photo ID will be required to verify identity.

Makeup Homework Policy
- Students are to make up all homework which was assigned during an absence. Students who are ill are entitled to full credit for make-up work. Generally, students are given the same number of days that they were absent to make up the work missed.
- 2.Requests for homework for absentees can be made when 3 days of absence are accumulated. Calls are directed to their Grade Level Office or the Magnet Office if the student is enrolled in the Magnet Program. Homework should be requested for long term absences only (e.g., more than one week). Homework assignments will be available in their Grade Level Office after school following the day on which the request is made. For short term absences, it is suggested that students know the phone number of a friend in each class, in order to obtain assignments. Teachers will also post assignments on the website: http://frostmiddleschool.org.
- Permit Information
OPEN ENROLLMENT
- Application Period: in May (see Announcements on Homepage, right column)
- Applications are available at all LAUSD schools.
- Parents should obtain an application from current school. Current school staff will sign in appropriate place.
- Submit application to school of choice by deadline.
- A lottery will be held on the day assigned by the District.
- Letters will be sent to all parents by June 10th. Letters of acceptance will include enrollment instructions.
- There is no school bus transportation provided.
SCHOOL FOR ADVANCED STUDIES
- Application Period: in May (see District Bulletin, April)
- Applications are available at all LAUSD schools.
- Parents should obtain an application from current school. Current school staff will sign in appropriate place and should also include the most recent test scores in the appropriate line.
- Submit application to school of choice by deadline with a copy of most recent test scores and either the letter of gifted eligibility or a letter of recommendation from the current teacher/coordinator.
- The S.A.S. Committee will review all applications for eligibility.
- Letters will be sent to all parents by June 10th. Letters of acceptance will include enrollment instructions.
- There is no school bus transportation provided.
CHILD CARE/WORK RELATED
- Applications are issued by Residence School.
- Submit application with documentation as required by the District and itemized on the back page of the application.
- Child Care Provider or Employer’s business address must be within the attendance area for Frost Middle School
Bus Transportation
- School bus transportation is provided to students enrolled in PWT, V/CAP, PSC or Magnet programs.
- Bus Routes are determined by the Area Bus Supervisors.
- Students are issued Bus Passes by the Area Bus Supervisor. Duplicates cannot be issued by the school.
- In the event that your child’s bus has not arrived at their bus stop on time, please call (800) 522-8737 to report the problem. You will need the Bus Route number and stop information.
- Any transportation changes should be directed to the Magnet Office for magnet students or their
Grade Level Office for all other transported students.
- Bus transportation is not available to students on Open Enrollment, School for Advanced Studies, or Child Care/Work Related permits.
Work Permits
Transfers to other schools
- All requests to withdraw from Frost Middle School must be in writing at least two days in advance. There is a Request to Withdraw form available in their Grade Level Office .
- All textbooks must be returned.
- All library books and fines must be taken care of at the Library.
- It takes two days to process the clearance form through all the classes and offices
- Upon completion of the clearance form, transfer papers (Pupil Accounting Report- PAR) will be issued to the new school.
- Transcripts/Cum Records will be forwarded upon written request from the new school.
- Enrollment Information
- All enrollments are to be done in the Grade Level Office your child is entering.

- Proof of residency (gas or electric bill in parent’s name or parent signs an address affidavit)/Prueba de residencia (el recibo de la cuenta de gas ó luz a nombre de los padres, o el padre necesita llenar un declaracion jurada para confirmar el domicilio)
- Most recent report card/El ultimo reporte de calificaciones
- Pupil Accounting Report (PAR) from current LAUSD school OR if current school is not LAUSD school, bring a birth certificate/Si el estudiante ha atendido anteriormente una escuela de Distrito Escolar Unificado de Los Angeles necesita solicitor el reporte llamado PAR y si nunca ha asistido a alguna de las escuelas de distrito tendrá que traer la partida de nacimiento.
- Individualized Education Plan if student receives Special Education services/Si el estudiante ha recibido servicios de educación especial, traer una copia del ultimo IEP
